secure merchant information form and will take you approximately
10 to 15 minutes to complete. After completing this form the information will be transfered to a Powerpays
and emailed to you to be printed, read, signed and faxed directly to the Underwriters for approval.

You will need to have the following information available:
Information: Legal and DBA (Doing Business
As) name, Business address, and name and title of legal
business signer, Business telephone number, fax number
(if applicable), contact name, E-mail address, and Internet
Physical location information, type of ownership, and
federal tax ID or SSN, date your business began and
what your business sells.
Data & Business Information: Total estimated
annual credit card sales volume and estimated average
ticket amount, credit card refund/return policy, manufacturer
of products (if any) and order fulfillment process information.
credit card processing information: Name,
address and phone number of current bankcard provider,
merchant number (if any).
Owner Information: Names, addresses, phone
numbers of first/second owner, partner, or officer,
Title and SSN of person(s) listed.
Information: Bank name and address, Contact
name and phone number and routing numbers (have a voided
check available for this information).

This is a combination Gateway/Shopping system.
